Guidance For 5 - 17 Year Old Patients:
- All minors must be accompanied by a parent, guardian or authorized adult. This adult is required to sign a consent and attestation form at the time of the appointment.
- Pfizer emergency use authorization (EUA) is authorized for ages 5+, while Moderna and J&J are authorized for ages 18+ (based on EUA and standing order which are subject to change).
- Parent, guardian or authorized adult will need to be present for the minor’s first and second COVID 19 Pfizer vaccine.
