Healthy Living: Self management of Chronic Conditions Workshop
YCCHS offers free Health Living with Chronic Condition workshops at various times and places throughout the year. There are three workshop types. Healthy Living with Chronic Conditions is for anyone with an on-going health condition and/or their caregiver. The other two are specific to a certain condition; Healthy Living with Chronic Pain and Health Living with Diabetes.
How was the program developed?
These workshops were developed by Stanford University and have been proven to help participants with an on-going health condition to learn how to better control their health. Workshop topics include healthy eating, dealing with difficult emotions, working with your healthcare team, and goal setting. The Healthy Living program lasts for 6 weeks and meets once a week for 2 1/2 hours. There is a break in each session. The workshops are free and interactive. Healthy snacks are provided.
